This option allows you to order gift invitations that will allow your recipients the opportunity to select their own gift choice from the Mayo Clinic $15.00 catalog selection.
When ordering, please select the quantity as the number of recipients you wish to award. Once you have completed your order, please fill out and email the Mayo Clinic Recognition Gift Shop Order Submission Form (located on the Mayo Clinic Recognition Platform) to the Mayo Clinic Store email address at mayoclinicstore@incentiveservices.com. Once your order confirmation and complete recipient list has been received, the opportunity to select gifts from the Mayo Clinic $15.00 catalog selection will be sent to your team.
